By entering this information online or calling in advance of your visit or pre-admission testing appointment, you allow us the opportunity to pre-authorize your visit with your insurance carrier to prevent authorization penalties. |
| How will I know if my online pre-registration form has been received? |
You will receive a reply via email once our Admissions and Registration Department is in receiptof your information. If additional information or clarification is required, a member of the Admissions and Registration Department will contact you by phone. |
